The Role of Clinical Trials in the Development of Social Medicine

Clinical trials (CTs) have traditionally focused on evaluating the efficacy and safety of medical interventions in controlled settings. However, the gap between their results and practical application in healthcare systems in different countries commands the need for better integration of research and clinical infrastructure. The objective: to analyze the modern approaches to the integration of CTs into the health care system, the role of pragmatic and implementation research. Materials and methods. The bibliosemantic method to identify the most reliable evidence for making political decisions regarding the organization of CTs on public health was used in the study. The study design included population-scale health impact assessments and considered economic and organizational implications. Results. CTs play a key role not only in the creation and verification of new medical interventions, but also in the development of social medicine – a discipline which is aimed at the interconnection between population health, social determinants and public health policy. The article summarizes modern approaches to the integration of CTs into the health care system, the role of pragmatic and implementation research, ethical and regulatory requirements, the connection with evidence-based policy and mechanisms for increasing the social impact of CTs. Recommendations are given to strengthen the impact of CTs on social medicine, taking into account modern methodological and technological capabilities. Conclusions. CTs are now not only a means to apply new effective treatments or procedures, but also an important tool for the development of social medicine. Pragmatic, implementable and embedded designs, integration with actual clinical evidence, ethical and regulatory modernization, and attention to social determinants provide the opportunity to transform research findings into measurable public benefits. This requires systematic collaboration between research institutions, health services, policymakers and communities.
